As part of the referral process to the PREP Program in Utah, we use the PRIME screening tool.
This tool has 12 questions that ask about a young person’s experience. The youth/young adult can fill this questionnaire out on their own, or you can read it to them and document their answers. Once you complete this tool, please reach out if there are any items where the young person answered with a 5 or 6.
